RELEASE FORM

The 2009-2010 Release Form is good for all trips during the 2009-2010 calendar year starting in September 2009 and ending in August of 2010.  For any trips outside of Loudoun County this form must be completed.  Please download the form and return it to the Youth Office, Leesburg Community Church.
